Structuring an enterprise-wide remote patient monitoring strategy at Jefferson Health

In the fall of 2020, Jefferson Health received a government grant to grow their remote patient monitoring (RPM) program, resulting in a unique opportunity to reduce cost of care while providing more convenient and continuous care for their patients through RPM — and a challenge to quickly deploy hundreds of RPM devices to patients across the 14-hospital system. While delivering these devices to patients, Jefferson recognized the need to develop an enterprise-wide remote patient monitoring strategy and find a technology partner to assist in its execution, all while responding to COVID-19. Bringing AVIA to the table enabled Jefferson Health to structure an enterprise-wide RPM strategy in just six weeks. Here’s what it took to get there.

Setting the foundation: Requirements for partner selection

Through their AVIA Membership, Jefferson Health engaged the AVIA team in an advisory project to support their RPM technology partner selection process. Given the breadth, reach, and goals of its RPM program, it was clear Jefferson would need a technology partner that would go beyond a traditional vendor offering. AVIA worked closely with a cross-functional team of Jefferson leaders, including the Chief Population Health Officer, Chief Operating Officer, Chief Medical Information Officer, and Vice President of Connected Care Operations, to identify their specific system needs and align on solution partner and solution functionality requirements.

Developing an RPM strategy with AVIA

In just six weeks — even amidst the constraints caused by the pandemic — AVIA helped Jefferson Health develop their enterprise-wide RPM strategy to focus on finding a partner they could co-develop with, not just a traditional vendor.

AVIA facilitated stakeholder interviews across the enterprise and aligned the Jefferson Health team around service line-specific goals, as well as short-, mid-, and long-term goals for the entire organization.
